- How do I get a copy of my medical or immunization records?
- Due to new federal regulations (HIPAA) we can not release your information without your written permission. This means x-ray reports, lab results, or copies of your medical record cannot be sent to your family doctor, hometown specialist, or your parents, without your written permission.
If you would like to make a request for your medical records, please download this form and fax it to 919-681-2874. Please document the exact information you are requesting. This policy does not apply to immunization records.
To request a copy of your immunization records, please send an email to DSHS_Immunizations@mc.duke.edu. We will send you a copy of your immunization records on file, but we will not complete your immunization forms for you.
