- How do I submit my Immunizations and Health History?
The Health History and Immunization Forms are now online. You will need your Duke Net ID and password which you will receive from OIT in your Duke Welcome Packet.
IMPORTANT: Before accessing the Health History and Immunization Online Forms:
1. Collect your immunization data. You may utilize official information collected from your medical provider's office, your high school or undergraduate institution, or the health department from the county where you received your immunizations.
2. Assure that your records show a minimum of 2 MMR (Measles, Mumps, Rubella) and 3 Tetanus inoculations. Your MMR's must have been given after 12 months of age and your most recent Tetanus must have been given after September 1, 1996.

Organize your information so you can complete your form in a timely manner. Be sure to enter your most recent inoculation date in the last date box.SPECIAL NOTE: Your form must be completed before you press the "Submit" button. If you press "Cancel" you must re-enter your immunization information. Once the form is submitted, you will not be able to access it again.
After you have gathered all of your immunization data as noted above:
1. Access and complete the Health History and Immunization Forms.
2. Verify that your form has been submitted successfully by re-accessing the link and verifying your Immunization Forms and Health History Forms are showing as "Completed."
3. In addition to submitting the electronic Health History Form you must fax or mail a copy of your immunization record which verifies the information you submitted online. The documents must include your full name and date of birth. Information must be faxed to 919-681-7386 or 919-681-2874 or can be mailed to:

Important Notes:
Duke immunization requirements are set by the State of North Carolina. Your state or country of origin may have different requirements. You must comply with North Carolina requirements to attend Duke University.
Re: MMR: You must repeat the Measles (Rubeola) vaccine if the first dose was received even one day prior to 12 months of age. History of physician-diagnosed measles disease is acceptable but you must submit a signed statement from a physician.
Re: Tetanus: Three dates from your primary series and a booster within the past 10 years are required. If you cannot document your primary series you must complete an adult series.
Screens/Titers: If you have positive antibody screens or titers, you must send copies of official lab results to verify immunity.
Tb: If you have a positive tuberculosis skin test, you must send a copy of the official X-ray report (the report is sufficient - please do not send X-ray films!)
Other: If you have received immunizations that are not requested on this form, you may provide copies for your record.
DEADLINES:
Forms must be received no later than:

* for Summer admission: April 15
