Matriculating in Fall 2009?

You must enter your health history and immunization information online. Log in using your Duke Net ID, then click on the "Forms" link on the left menu bar. Duke's required immunizations are those required by North Carolina Statutes; therefore, you must prove your immunization compliance in advance of entering classes. Please review the requirements and instructions for form completion(pdf) carefully and complete the forms as soon as possible so that we can follow up with you if needed before you arrive on campus.

In the navigational bar to the right you can find some forms you may need while at Duke.

• Clinical forms: To save time during a visit to the Health Center, here you have the option of downloading and printing a form prior to your visit.
  
• Health Promotion: For students interested in or already involved in the Peer Education program.
  
• Policies: Detailed information on the Student Health Fee, Medical Insurance, HIPAA, and other important areas.
