Take an Online Self Screening
Duke Student Health welcomes you to the online screening for eating disorders, alcohol, and depression. This free screening is made available to all Duke Students and is taken anonymously. Although we confirm that you are a Duke Student by having you log in with your Duke NetID, the data you enter is in no way linked to your log in and is completely anonymous. You may take the survey once per semester.
This screening is intended to provide you with feedback in regard to any concerns you might have with eating behaviors, alcohol consumption and/or depressive symptoms or episodes. These results do not constitute any type of diagnoses, but are intended to provide you with insight into what you feel may be a problem for you. Should you care to speak with a health professional about your results or any concerns regarding these matters, we encourage you to call 681-WELL to schedule an appointment with a dietitian for eating behavior concerns and CAPS at 660-1000 for matters concerning alcohol or depressive symptoms or episodes. You can also meet with any health care provider to discuss these issues if you have a provider that you feel very comfortable with.
If you are ready, you may proceed.
