Welcome to Duke. Below you will find links to information that may help your transition to Duke.
- Health History and Immunization Requirements: Please review the requirements and instructions for form completion(pdf) carefully and complete the forms as soon as possible so that we can follow up with you if needed before you arrive on campus. You must enter your health history and immunization information online. Log in using your Duke Net ID, then click on the "Forms" link on the left menu bar. Duke's required immunizations are those required by North Carolina Statutes; therefore, you must prove your immunization compliance in advance of entering classes. For first year students living on campus, you will not be able to receive your room key until you have completed this requirement. If you have submitted your health history form and immunization documentation, we will only contact you (through your Duke email) if there are problems, otherwise you can assume you are in compliance. - 2009-2010 Health Insurance: All Duke students are required to have medical insurance - either through the Duke-sponsored plan or a comparable medical insurance plan based in the United States. If you are arriving January 1, 2010 or later, your may enroll in the current Duke Student Medical Insurance Plan (SMIP) which will be through United Healthcare Student Resources.
The Duke Student Medical Insurance policy year will run from August 1, 2009 to July 31, 2010. If you are a new student, please contact your department or school if you are required by Duke to be on campus before the start of the policy and will not have any other coverage.

If you are arriving at Duke for school related activities prior to the start of the 09/10 Duke Plan year and need to secure early insurance coverage (for individual and/or dependent plans), you may be eligible to enroll in the remaining portion of the 08/09 Duke SMIP.

If you hold a F-1 or J-1 visa, participation in the Duke SMIP is mandatory.

- Health Fee: All currently enrolled full-time students and part-time degree candidates are assessed a mandatory student health fee each semester. This fee supports a number of services and activities that foster a healthy campus community. The fee covers most services rendered within the Student Health Center (SHC) during each enrolled semester, while other services may be covered by the Health Insurance. - Medical Services: The Duke Student Health Center is the primary care facility for all Duke students. A wide range of services are available at Duke Student Health, including general medical care, women's health care including gynecological exams, men's health care, nutrition counseling, sports medicine consults, nurse consults, allergy clinic, and travel clinic. There is a laboratory in the Student Health Clinic and a pharmacy on the first floor of the Duke Clinic building. For more information, please visit the Medical Services section of this website.
